Payment for services is due at the time services are rendered unless arrangements have been approved in advance by our staff.
 
We accept cash, and all MAJOR CREDIT CARDS. We will be happy to process your insurance claim form for you.
 
Provide us with all the necessary information of the insured and we will accept assignment of the insurance benefits. IN ALL CIRCUMSTANCES THE PATIENT IS RESPONSIBLE FOR ALL COSTS NOT PAID FOR BY THE PATIENT’S INSURANCE COMPANY.
